1, If you are applying for China visa at the very first time. Please submit your last Original Chinese Passport (If you cannot provide it, we cannot process your application).
2, If you are Not applying at the very first time and your old China visa is not on your current passport. Please submit a photocopy of your former passport (photo page only) and the former China visa page.
3, If you are Not applying at the very first time, and your last Chinese visa is on your current passport, you do not have to provide any additional document.
- If you did change your name in the process of marriage, naturalization or other reasons), please provide the name change proof. (It is a court order confirmation, photocopy only)
- All supporting original document such as Chinese passport will be return to the applicant when the visa issued.

b Photocopy of USA birth certificate (if any)
c Photocopy of foster parents’ passports
d Child’s original Chinese Passport (not photocopy)
e Adoption Certificate and any other support document (photocopy only. Normally, there are two Adaptation Certificate, one was issued by Chinese government, and another one was issued by USA government. Normally, this is the name change proof as well.)
f Child’s naturalization certificate (photocopy) , or green card front and back (photocopy)
2, If you are Not applying at the very first time and your old Chinese visa is not on your current passport. Please submit the photocopy of your old passport (photo page only) and last Chinese visa page, and everything listed on No1, except Child’s original Chinese Passport.
3, If you are Not applying at the very first time, and your last Chinese visa in on your current passport, Please submit everything listed on No1, except Child’s original Chinese Passport.
